Family Medicine and Specialty Services

The Laurel Health Centers provide award-winning family medicine, mental health, and specialty care to patients of all ages. We're Federally Qualified Health Centers (FQHCs), also known as Community Health Centers (CHCs), which means we treat patients even if they cannot pay. We provide a sliding fee scale to income-eligible patients to assure everyone can access the care they deserve whenever they need it.

At Laurel Health, you're more than just a patient. We treat everyone we see as individuals with unique needs instead of relying on a one-size-fits-all approach to healthcare. We take a holistic, whole-person approach to care, focusing on proactive, preventive services and seamless care coordination. Whether you need pediatric care, women's health services, dental care, or behavioral health services, we're here for you. Always.

Find great care close to home at our convenient clinics, strategically located across Bradford, Tioga, and Sullivan Counties in northcentral Pennsylvania:
