Insurance Coverage - Medical Health Plans
Laurel Health accepts Medicare, Medicaid, and most major commercial insurances, including plans from:
- United Healthcare
- Humana
- AmeriHealth
- UPMC
- Geisinger
- Blue Cross
- Highmark
We also accept sliding fee coverage, Medicaid, Medicare, and the Children's Health Insurance Program (CHIP).
Are you uninsured or underinsured? Our certified application counselors can help you find a plan that is a good fit for your needs. We can also help you navigate the online insurance exchange and enroll in healthcare coverage. We offer free enrollment assistance for Medicaid and commercial healthcare.gov plans. Our team can take some of the stress out of choosing a healthcare plan by providing a free, unbiased comparison of health insurance options on the PA insurance marketplace, Pennie.com.
These services are offered free to our community; you do not have to be a Laurel Health patient to schedule a free consultation with our enrollment counselors. Insurance Coverage - Dental Plans
Laurel Dental participates in a wide array of dental insurances. We currently participate with plans by the following dental carriers:

- Medical Assistance (Medicaid and Children's Health Insurance Program)
Our dental team is always available to discuss payment concerns, options, and plans. We help patients review / confirm coverage, and we offer a sliding fee program to help income-eligible patients with healthcare costs. Laurel Health's free enrollment counselors are also available to assist families with reviewing their eligibility for additional coverage and financial assistance.

The Benefits of Using In-Network Providers
Many health insurance plans have different levels of coverage based on whether facilities and providers are considered “in network.” This term refers to doctor's offices, hospitals, and healthcare professionals your insurance plan contracts with to provide care to its members. Laurel Health participates with and accepts a wide variety of health insurance carriers, including both medical and dental plans.
There are many benefits to selecting an in-network provider for healthcare services. Some of these advantages include:
- Lower costs: At an in-network provider, services are provided at a lower rate that's been determined by your healthcare plan. Choosing an in-network provider results in lower overall healthcare costs.
- Deductible expenses: Fees you pay at an in-network provider count toward your out-of-pocket maximum and annual deductible. Some insurance plans have separate deductible amounts or limited coverage for out-of-network care.
- Accurate and efficient claim processing: The details for in-network providers and facilities are already on file with your insurance plan, which makes for quick and accurate claim processing.
How to Pay Your Bill
If you have insurance coverage, your claim will be processed through your insurance company before you receive a bill. Should you owe additional payment following your insurance's reimbursement, you will be asked to remit payment to our billing office. If you have a question about how insurance covered your visit, please contact your health insurance company directly. If you have a question about your bill, call us at 1-877-723-0589.
You can pay your bill by returning the payment voucher by mail, in person at your Laurel Health office, or over the phone by calling the phone number listed on your bill. Laurel Health does not currently offer online bill pay.
Co-pays for office visits are due at the time of your appointment. We accept payment by cash, credit card, debit card, or check.
Our Sliding Fee Program
Laurel Health is comprised of Federally Qualified Health Centers (FQHC), also known as community health centers, and we offer our medical services to everyone, even when they cannot afford it or do not have insurance.
To ensure all patients maintain access to affordable, high-quality healthcare, Laurel Health offers a sliding fee program. This program allows our offices to slide the fee for our services based on income eligibility guidelines. The sliding fee program is offered for all Laurel Health locations, including the Laurel Health Centers, Laurel Behavioral Health, and Laurel Dental clinics.
Our team is committed to making sure everyone receives the care they need. Our sliding fee program helps income-eligible patients with co-pays, deductibles, and medications. We also help patients evaluate their eligibility for additional financial aid like the CHIP, Medicaid, Medicare Extra Help, and Medicare Savings programs. We can also assist you with financial aid applications and insurance enrollment.
The sliding fee program is designed to make care more affordable for patients with limited income. This program uses the Federal Poverty Guidelines (FPG) to define a patient's income eligibility. The FPG guidelines are updated annually. Your size of your family (household) and your annual income are used to determine your program eligibility.
Based on your eligibility, the program then slides healthcare fees based on the rubric defined by the FPG. Individuals or families with annual incomes at or below 100% of the FPG standard may pay only a nominal fee for healthcare services. Patients above the 100% FPG standard but below 200% may receive partial discounts on costs. For full details, please speak with our billing office at 1-877-723-0589.
Keeping Your Information Safe
Laurel Health takes patient privacy very seriously, including safeguarding your personal, clinical, and financial information. You can expect open and transparent communication about how and when we access your information to provide care. If you have any questions about your patient information or would like to request a copy of your medical record, simply call your Laurel Health office, and we're happy to help.
Your Health Information
Laurel Health cares about your health and well-being, including your privacy. We take care to keep your insurance information and other patient data safe. Our health system follows industry best practices and the Health Insurance Portability and Accountability Act (HIPAA) to safeguard your privacy.
You can use our patient portal to securely review your medical history, renew prescriptions, schedule appointments, and contact your provider. This platform, developed by the University of Pittsburgh Medical Center (UPMC), follows stringent security protocols to ensure your data is secure.
Despite the name “MyUPMC,” this portal is not exclusively used by UPMC facilities or UPMC patients. It is also used by other, unaffiliated health systems. Why? Many rural health systems opt to participate in an existing patient portal platform instead of creating and hosting their own due to the technical demands, costs, and intense security protocols required.
Laurel Health previously used the MyKeyCare portal, which was created and hosted by Geisinger Health System. These patient portals are used to provide a secure, robust platform for our patients. Laurel Health is not affiliated with or part of UPMC or Geisinger.
