Clinical Specialties & Areas of Focus: 

Hana Sullivan, Adult Gerontology CRNP (AG-PCNP) is a registered certified nurse practitioner trained with a focus on primary care and adult gerontology care. She brings additional experience in pulmonology, emergency care, long-term skilled nursing facilities, and inpatient settings. She will be seeing patients of all ages but specializes in gerontology, particularly older adults with chronic conditions and healthy aging.

About Hana Sullivan: 

“I believe that timely, quality care should be guaranteed for everybody, but that can be especially challenging for people living in remote rural areas,” she shares. “I care for patients of all ages, but from the bottom of my heart, I’m passionate about caring for older adults who need specialized and compassionate care for chronic illnesses. One of the critical roles we have as healthcare providers is delivering primary and preventive care with patient education to promote well-being as we age. I am absolutely thrilled to join this team to help my community and my patients live better, happier lives!” 

In her free time, Hana enjoys exploring nature on walks with her busy toddler and recharging her energy with yoga, music, journaling, reading, baking, and crime shows. She loves adventure, trying new things, and traveling; one of her favorite and most memorable trips to date was a visit to Angkor Wat in Cambodia.

Education: 

Hana received her Associate Nursing Degree from Ulsan College in South Korea, her Bachelor of Science in Nursing from SUNY Binghamton in New York, and her Master of Science in Nursing with a special certification in adult gerontology from SUNY Binghamton in New York.
