Laurel Behavioral Health Care Coordination
Feeling overwhelmed by scheduling an appointment, managing a busy schedule, or juggling your family's healthcare needs? We can help. Navigating healthcare can be overwhelming and stressful sometimes, especially when you are managing multiple needs, symptoms, and complex conditions at the same time.
We have dedicated LBH care coordinators to assist our patients with accessing health services and community resources. Our care coordinators can help you manage appointments, referrals, and information to keep your care team on the same page. Care coordination supports easier care transitions across Laurel Health and other organizations to help patients better access care and stay on track with their personalized care plans (e.g., if an LBH patient also needs a dental or family medicine appointment, our care coordinators can help book those appointments to make the process easier).
Laurel Behavioral Health takes a whole-person approach to care, which means we look at how your mental, physical, emotional, and socioeconomic needs overlap and affect one another. For example, if someone is experiencing mental stress, it can cause physical symptoms, so an effective care plan needs to address both the physical and mental health needs to support overall well-being.
Our team focuses on treating all of your needs in tandem instead of just focusing on isolated symptoms. We also connect patients with helpful community resources that support overall well-being like supplemental food programs, transportation assistance, housing, clothing resources, patient education materials, and local support groups.
